Occupational Health Form

Post by londonlad81 » Tue May 26, 2020 3:32 pm

Hi guys,
Today I got the OH form emailed to me by Optima health.
It asked me about immunisation history, for varicella.
I had to check what this is and it’s chickenpox.
I had this as a kid, but wasn’t immunised against it and can’t remember when this was. As far as I remember, it lasted a few days and that was that.
It asks for the history date before you can move onto the next question. Any had this issue ?

I rang my gp surgery to ask for a print out of my immunisations, I was pleasantly surprised how quick they were at sending me it. Nothing about chicken pox is on there ( I know I didn’t have an injection) but for my date of birth nothing was recorded in it, infact the record only started when I was nine and that was the MMR.
JCOX, did you actually have the varicella jab or did you just put your date of birth down to answer the question?
The rest of the OH form was fairly simple with yes and no questions.

I called the doctors and asked when I had chicken pox because I remember going for a diagnosis. So I had that date and I put that on my form because you rarely get them twice!

LondonLad, did you have to pay for them to provide you with this? I’ve just done the oc health online form and this varicella vaccine is new to me. Didn’t even know there was one, thought chickenpox was just something that you either got or didn’t haha.

It automatically populated to today’s date so just left it. Even rang me mam and she had no recollection of me ever having a chickenpox vaccine. I’m sure they’ll let me know if there’s an issue 🙄

The immunization history you receive from gp is unlikely to go back that far anyway. No harm in making it up

No I didn’t pay for anything from the GP.I telephoned the surgery and they emailed me my immunisation history. They were surprisingly helpful!
I was stuck on that varicella question but know it’s chicken pox. I did ring the occupational health team for advice ( sorry can’t remember what they said!).. The lady may have said just put today’s date but not certain..

I remember having chicken pox as a child but don’t think I was vaccinated against it, don’t think many children are.

Cheers mate. They’re emailing mine over anyway, just so I’ve got them incase they query anything. Think it’s a strange one, could understand if we had a Covid vaccine and they wanted to know if you’d had that but chickenpox? :?

It can be fatal in adults, maybe they have to cover their arse in case there's a prisoner that comes down with it

Ring the occupational health form if you’re unsure about anything, but definitely worth getting a print out of you’re immunisation history, just incase they do check further down the line..not sure they would though.
Get the form done ASAP mate, another box ticked!
