when on long term sick, do you have to have a home visit or is it something that you can refuse?

A home visit is usually required however you can agree to meet at an alternative venue ie: cafe, park or somewhere mutually agreed with no obvious distractions.

You can refuse to see your manager but not advised, your case whatever, diminishes, they could even stop your pay for not abiding to policy.

They can't stop your pay for refusing a home visit.
You are within your rights to decline the visit and opt out of any visit what so ever.
I'll have to do so more digging on this subject, if you want to go down that road, in box me and I'll do my best to get the info for you.
I wouldn't advise it though, if you play 'silly beggers' with them, they WILL do it right back 10 fold.
