I used my partner’s friend as mine and my vetting passed all ok, you should be fine

I did my fitness in February, I wasn’t asked to take any forms with me as they had them there to fill out which I assume was the health questionnaire, no idea what the consent form could be
Only needed to show my photo ID but took passport/birth certificate just incase
